fMRI Full Form: The full form of fMRI is functional magnetic resonance imaging. It is a scan that measures and maps the activity of the brain. An fMRI scan uses the same technology, just like an MRI scan. An MRI is a noninvasive scan or a test that uses a strong magnetic field and radio waves to create an exact image of the brain. The image produces by an MRI scan is just the image of organs/tissue, but in the case of fMRI, it will produce an image showing the blood flow in the brain. Also, it will display which parts of the brain are being stimulated.

Principle of fMRI

  • The main method used to generate images is contrast imaging.
  • Haemoglobin in the blood carries oxygen; oxyhemoglobin around the brain, and when it is used up, it becomes deoxyhemoglobin, where the oxygen is used up, showing the activity site in the brain.
  • The picture is made by monitoring the ratio of the tiny wave frequencies between these two states whilst the patient carries out a task, e.g., Tapping a finger, which highlights the area of the brain functioning to carry out this task.

What does the equipment look like?

The standard MRI machine consists of a sizable tube with a circular magnet surrounding it. You will be placed on a table that slides into a tunnel leading to the magnet’s core.

Some MRI machines, known as short-bore systems, are made to only partially enclose you in the magnet.

How is the procedure performed?

MRI tests can be performed in an inpatient setting.

You will be positioned on the mobile exam table by the technologist. To help you remain still and hold your position, they may use straps and bolsters.

The technologist may put objects with coils that can transmit and receive radio waves close to or around the area of the body being examined.

The majority of MRI exams consist of several runs (sequences), some of which can last for several minutes. Different sounds will be produced during each run.

Your head might be put in a brace made to hold it still during the fMRI scan. This brace might come with a customized mask for you. To protect you from audio-visual stimuli, you might be given specialized goggles and/or earphones to wear. (For instance, a computer screen projection or recorded sounds) could be used during the scan.

A doctor, nurse, or technologist will place an intravenous catheter (IV line) into a vein in your hand or arm if your exam requires contrast material. The contrast agent will be injected using this IV.

You will be positioned inside the MRI machine’s magnet. The examination will be conducted by a technologist while they are seated at a computer outside the examination room. The technologist will be reachable via an intercom.

The technologist may ask you to wait until the exam is finished while the radiologist reviews the images in case more are required.

After the examination, the technologist will remove your IV line. and cover the insertion site with a small dressing.

The entire exam is typically finished in an hour.

MR spectroscopy may also be used by the doctor during your examination. More details on the chemicals found in body cells are revealed by MR spectroscopy. The total exam time may increase by about 15 minutes as a result.

Comparisons Done by fMRI with Quantitation

Several methods can be used for fMRI comparisons

Different oxygen usage levels can be determined with a pseudo-color scale in the images acquired.

Example-

  • Four-color scale
  1.   The red color indicates the highest level of oxygen uptake
  2.   An intermediate level of uptake of oxygen is indicated by yellow color
  3.   The normal level of oxygen is shown by green color
  4.   Lower than normal level of oxygen is indicated by blue color

 The volumes of the differently colored regions in the brain can be determined by using these types of color scales.

Comparisons Done by fMRI with Qualitative Evaluation

  • During a particular type of movement, when a brain gets activated can be determined by fMRI.
  • The activated areas during any activity might be nonspecific, so in that case, investigators must carefully consider each site of activation to determine.
  • To do so, the investigator must take at least 6 individuals for accurate comparisons between the activities of different individuals’ brains.

Measurement of Brain Activity by fMRI

According to the FMRI, there are certain brain measurements based on the activities performed such as-

Advantages of fMRI

There are several benefits of the fMRI, some of them are as such-

  • fMRI helps map a patient’s brain before they go into brain surgery.
  • fMRI map helps doctors understand the brain’s regions linked to critical functions such as speaking, walking, sensing, or planning.
  • the scan provides about the makeup of an individual’s brain to prevent serious injuries.
  • It also determines if surgery is even possible or not.
  • Another advantage is that the image produced by the fMRI scan is very high resolution.

Disadvantages of fMRI

There are several disadvantages of the fMRI, some of them are as such-

  • fMRI studies of brain cancer, lesions, and other brain pathologies of humans and animals are still to be explored.
  • Using fMRI, we face difficulty in cognitive studies interpretation. This can only be overcome by imaging large numbers of samples
  • Getting an fMRI scan is a costly procedure.
  • A minor negative is that a machine can only capture a clear image if the person being scanned is completely skillful.
  • only the blood flow can be looked not at individual neuron activities.

What are the limitations of fMRI?

You must be able to hold still and follow breath-holding instructions while the images are being taken in order to get high-quality pictures. It might be challenging for you to lie still during imaging if you’re feeling anxious, disoriented, or in a lot of pain.

Head motion is particularly detectable by fMRI. It’s crucial to remain calm and motionless throughout the scan.

You must be able to complete the tasks given to you in order to use fMRI. You should inform the technologist if the tasks are too challenging for you to complete or if you are having trouble carrying out the actions. You might receive alternative assignments or simpler quizzes.

Certain MRI machines may not be able to accommodate a person who is very large. The scanners have weight restrictions.

It can be challenging to get clear images when there are implants or other metallic objects present. The same result can be achieved by patient movement.

Images may be impacted by a heartbeat that is extremely erratic. This is due to the fact that some imaging techniques base their timing on the electrical activity of the heart.

For patients who have sustained serious injuries, MRI is typically not advised. But this choice was made using clinical judgment. This is due to the possibility of MR images being distorted by traction devices and life support equipment. They need to be kept away from the imaging area as a result. However, some trauma patients might require an MRI.

A pregnant woman’s fetus cannot be harmed by non-contrast MRI, according to the available data. Your doctor may postpone the exam until after delivery if the need for it is not time-sensitive. Except in very limited situations, gadolinium contrast agents for MRI should generally be avoided during pregnancy.
