Documents Required for UPPSC Polytechnic Lecturer DV

  1. Applicants will be required to provide four passport-size photographs, two non-certified and two certified by the Head of Department or Head of Institution where they receive education or the Gazetted Officer during the Dialogue.
  2. Matriculation/10th Standard or equivalent issued by the Central / Government Board showing the date of birth to support their age claim.
  3. Degree certificates along with mark sheets pertaining to all the academic years as proof of educational qualification claimed.
  4. The Experience Certificate if any must be issued in the prescribed manner appropriate to the post.
  5. Caste Certificate of the person who wants to be booked as SC / ST / OBC, in a prescribed profile from competent authorities.
  6. All the UP residents should also furnish the Domicile certificate.
  7. A declaration in the prescribed format of the person who wants to be booked as an OBC, that he is not a layer cream on a special day, in addition to a public certificate (OBC).
  8. Applicants working under the Central Government or the State will have to produce 'NOC (No Objection Certificate)' to their employer during the consultation process provided by the relevant authority.
  9. The Competent Authority to issue a Physical Disability Certificate shall be a Medical Board duly appointed by the Central or State Government.
  10. An aspirant who claims change in name after 10th on marriage or remarriage or divorce etc. the required documents shall be submitted at the time of interview
  11. Age relaxation Certificate/ Document issued by the competent authority required for the candidates who want to seek age relaxation.
